(HI) Shared decision making (SDM) is a collaborative process that allows the patient and provider to make important health care decisions together. It takes into account the best clinical evidence available as well the patient's preferences for care. Patient decision support tools, also known as decision aids, are evidence-based tools used to help patients understand their condition and the preference-sensitive decision that needs to be made.

Implementing SDM into routine clinic processes can be challenging. During this presentation by Megan Bowen, compliance specialist and site visitor with the Oregon Health Authority's Patient Centered Primary Care Home Program, you'll learn about some valuable lessons from the field, how to choose the most appropriate SDM topics for your clinic, how to integrate patient decision support tools and the process of SDM into routine workflows, training resources for providers and staff, and where to get additional resources.

Meaningful Use Education Series: Clinical Quality Measures: What Are They and Why are They Important to You and Your Patients?

The HealthInsight Regional Extension Center and Quality Innovation Network-Quality Improvement Organization are coming together to present a joint conference on Clinical Quality Measures for eligible professionals. This collaboration will serve as the foundation for future Learning and Action Network (LAN) sessions in 2015.

Clinical quality measures (CQM's) are synonymous with the Physician Quality Reporting System (PQRS). Because there are hundreds of measures to choose from, picking the right ones can prove to be confusing. This session will also cover the importance of being able to report only once and yet, meet multiple reporting and incentive program requirements. Combined with the different ways that one can be rewarded for meeting these requirements makes attending this session even more important as you prepare for 2015 reporting!

Topics covered include:
1. What are CQM's? Domains and measures
2. Why are they important? Financial incentives and disincentives
3. Useful strategies to select measures
4. Deadlines to avoid penalties
5. Question and answer period.

Please join us on December 17th for what will be a timely and important session as you prepare your practice for 2015. Please bring your questions and concerns to share with others in our community.
