All beneficiary counts in this report include only Medicare FFS beneficiaries with diabetes as determined from Medicare claims for 12 months ending 6/20/2017. Beneficiary counts less than 11 are reported as 11.
Socioeconomically Deprived Area Residents: Socioeconomic status is determined by Centers for Medicare & Medicaid Services using the Area Deprivation Index (ADI) for the zip+4 area in which the beneficiary resides. The ADI uses a combination of individual and family income, education level, occupation and other related factors. Disparate Beneficiaries: Disparate beneficiaries in this report takes into account populations (based on ZIP code) that are in rural or socioeconomically deprived areas, or Medicare beneficiaries who are Hispanic or of a race other than Caucasian, or individuals who have both Medicare and Medicaid insurances (dual eligible).
AADE and ADE Facilities: Facilities with diabetes self-management training/education programs accredited by the American Association of Diabetes Educators or recognized by the American Diabetes Association.
