(HI) New models for community-clinical partnerships are emerging using community health workers to bridge care gaps between community and clinical services, which has increased access to appropriate health care, self-management education and social services for underserved populations. Payment reform provides outpatient practices with new opportunities to structure care that better addresses the social determinants of health and patient activation. This webinar will share two models that rely on clinical-community partnerships to promote patient activation for rural, underserved and cultural-specific communities.

(HI) The Quality Assurance Agency (QAA) team at Jefferson Community Health and Life - Gardenside LTC, in Fairbury, Nebraska, implemented Natural Awakening to improve the overall sleep hygiene, health and safety of their residents, while promoting a more \"home-like\" environment. In this presentation, they will share the steps taken, lessons learned and challenges they have faced through their journey of Natural Awakening.

Webinar Objectives: Participants will be able to:
• Recognize health and safety concerns related to fragmented sleep patterns
• Identify the top disturbances of sleep
• Identify barriers to implementing Natural Awakening in their facility
• Describe steps to implement a successful Natural Awakening Program promoting uninterrupted sleep

Target Audience: The webinar is designed for nurses and other nursing home staff.

Presenters: Debra Sutton, RN, BS, NHA, has been a nurse for 34 years, with 18 of those in long-term care as the director of nursing. Three years ago she obtained her nursing home administrator's license and since that time has been the administrator at Gardenside LTC in Fairbury, NE. Deb and her staff work to promote resident-centered care at Gardenside and to that end, began the Natural Waking Program.
Samantha Wurm, LPN, began employment at Gardenside as a CNA in 2014. She became a Licensed Practical Nurse in 2016 and assumed her role as charge nurse. She worked closely with the QAA leader on different projects for Gardenside, then took over as QAA leader in in the fall of 2016.

Pre-work: We recommend participants view the June 6, 2017, webinar with Sue Ann Guildermann on \"Sleep Hygiene/Falls – Part II: Restorative Sleep Vitality Program.\" A good night of undisturbed sleep is one of the most important services we can give our residents. By eliminating or minimizing night-time interruptions, we can encourage more of the restorative sleep our residents need to maintain and enhance their quality of life. The presentation also provided practical strategies for preventing falls through uninterrupted sleep. Watch the recording now and/or review the presentation here.

(HI) Shared decision making (SDM) is a collaborative process that allows the patient and provider to make important health care decisions together. It takes into account the best clinical evidence available as well the patient's preferences for care. Patient decision support tools, also known as decision aids, are evidence-based tools used to help patients understand their condition and the preference-sensitive decision that needs to be made.

Implementing SDM into routine clinic processes can be challenging. During this presentation by Megan Bowen, compliance specialist and site visitor with the Oregon Health Authority's Patient Centered Primary Care Home Program, you'll learn about some valuable lessons from the field, how to choose the most appropriate SDM topics for your clinic, how to integrate patient decision support tools and the process of SDM into routine workflows, training resources for providers and staff, and where to get additional resources.

(NV) Why Reduce Readmissions?
Hospitalizations are among the most expensive forms of health care. The estimated annual financial impact of 30-day readmissions for Medicare alone is $17 billion. Evidence today suggests that a single hospitalization alone can contribute to further deterioration in patient health status, particularly for the elderly.

Meaningful Use Education Series: Clinical Quality Measures: What Are They and Why are They Important to You and Your Patients?

The HealthInsight Regional Extension Center and Quality Innovation Network-Quality Improvement Organization are coming together to present a joint conference on Clinical Quality Measures for eligible professionals. This collaboration will serve as the foundation for future Learning and Action Network (LAN) sessions in 2015.

Clinical quality measures (CQM's) are synonymous with the Physician Quality Reporting System (PQRS). Because there are hundreds of measures to choose from, picking the right ones can prove to be confusing. This session will also cover the importance of being able to report only once and yet, meet multiple reporting and incentive program requirements. Combined with the different ways that one can be rewarded for meeting these requirements makes attending this session even more important as you prepare for 2015 reporting!

Topics covered include:
1. What are CQM's? Domains and measures
2. Why are they important? Financial incentives and disincentives
3. Useful strategies to select measures
4. Deadlines to avoid penalties
5. Question and answer period.

Please join us on December 17th for what will be a timely and important session as you prepare your practice for 2015. Please bring your questions and concerns to share with others in our community.

The Centers for Medicare & Medicaid Services has now finalized the proposed 2014 Certified Electronic Health Record Technology (CEHRT) for meaningful use (MU) rule which gives providers some flexibility when attesting in 2014.

This rule takes into account the struggle electronic health record vendors have had in developing and rolling out 2014 certified versions and that physician offices and hospitals have had in implementation of newer versions. Join our webinar to learn about the flexibility and tools available for navigating meaningful use in 2014.
