(HI) Antibiotic resistance is a critical threat to our ability to combat infections—sometimes with fatal consequences, as in a recent Nevada case when no antibiotic could help the patient. As an approach for avoiding resistance, appropriate antibiotic prescribing has never been more important. But without access to an in-house pharmacist, how can a primary care provider know which drugs are best for specific conditions? This webinar describes the use of community tools including antibiograms—data-based profiles of response of various disease organisms in a community to specific antimicrobial drugs—in the development of antibiotic stewardship plans and outpatient prescribing considerations within communities.

(HI) The Quality Assurance Agency (QAA) team at Jefferson Community Health and Life - Gardenside LTC, in Fairbury, Nebraska, implemented Natural Awakening to improve the overall sleep hygiene, health and safety of their residents, while promoting a more \"home-like\" environment. In this presentation, they will share the steps taken, lessons learned and challenges they have faced through their journey of Natural Awakening.

Webinar Objectives: Participants will be able to:
• Recognize health and safety concerns related to fragmented sleep patterns
• Identify the top disturbances of sleep
• Identify barriers to implementing Natural Awakening in their facility
• Describe steps to implement a successful Natural Awakening Program promoting uninterrupted sleep

Target Audience: The webinar is designed for nurses and other nursing home staff.

Presenters: Debra Sutton, RN, BS, NHA, has been a nurse for 34 years, with 18 of those in long-term care as the director of nursing. Three years ago she obtained her nursing home administrator's license and since that time has been the administrator at Gardenside LTC in Fairbury, NE. Deb and her staff work to promote resident-centered care at Gardenside and to that end, began the Natural Waking Program.
Samantha Wurm, LPN, began employment at Gardenside as a CNA in 2014. She became a Licensed Practical Nurse in 2016 and assumed her role as charge nurse. She worked closely with the QAA leader on different projects for Gardenside, then took over as QAA leader in in the fall of 2016.

Pre-work: We recommend participants view the June 6, 2017, webinar with Sue Ann Guildermann on \"Sleep Hygiene/Falls – Part II: Restorative Sleep Vitality Program.\" A good night of undisturbed sleep is one of the most important services we can give our residents. By eliminating or minimizing night-time interruptions, we can encourage more of the restorative sleep our residents need to maintain and enhance their quality of life. The presentation also provided practical strategies for preventing falls through uninterrupted sleep. Watch the recording now and/or review the presentation here.

(HI) Shared decision making (SDM) is a collaborative process that allows the patient and provider to make important health care decisions together. It takes into account the best clinical evidence available as well the patient's preferences for care. Patient decision support tools, also known as decision aids, are evidence-based tools used to help patients understand their condition and the preference-sensitive decision that needs to be made.

Implementing SDM into routine clinic processes can be challenging. During this presentation by Megan Bowen, compliance specialist and site visitor with the Oregon Health Authority's Patient Centered Primary Care Home Program, you'll learn about some valuable lessons from the field, how to choose the most appropriate SDM topics for your clinic, how to integrate patient decision support tools and the process of SDM into routine workflows, training resources for providers and staff, and where to get additional resources.

(HI) You've submitted your 2015 data to CMS under requirements for PQRS or MU or another program. Your feelings are:
a. Thank goodness that is over and we don't have to worry about it until next year.
b. That was a nightmare! We're still trying to catch-up on other work.
c. Frustrated because we found out CMS doesn't accept the files that our EHR produces.
d. We found some data gaps that need to be addressed in the next couple of months and are already updated our policies and procedures.
e. That was easy! We were ready to submit advance because of all of our prep work.

If your answer is anything but e) then this webinar is for you! Join us for a discussion on assessing your practice's readiness and developing a workflow to ensure data is entered appropriately to produce your needed reports.
