Alzheimer's Disease and Hospital Readmissions: Policy Brief on Prevention
The Alzheimer's Association has published a policy brief that outlines reasons more than 25% of people living with Alzheimer’s or other dementias experience preventable hospitalizations. These hospitalizations cost Medicare nearly $2.6 billion in 2013 and were associated with poorer health outcomes for the people involved. Healthy People 2020, our national public health agenda, includes an objective to "reduce the proportion of preventable hospitalizations in persons with diagnosed Alzheimer’s disease and other dementias."
The brief identifies approaches that can potentially help reduce this high incidence of readmission, including improvements in primary care services and disease management for community-dwelling individuals living with Alzheimer’s and other dementias.
