2017 Learning Modules for Oregon Primary Care Home StandardsThe Patient-Centered Primary Care Institute, in partnership with the Oregon Health Authority, has launched the updated 2017 Online Learning Modules for the Patient-Centered Primary Care Home (PCPCH) Standards for Recognition.
These modules are an essential educational resource for the Oregon Health Authority's PCPCH model, Oregon’s version of the medical home for primary care home redesign. An audio-visual companion to the PCPCH Technical Specifications and Reporting Guide, the modules describe the intent of the Standards, as well as practical implementation advice and examples.
Modules may be accessed by anyone at any time after answering a few registration questions. Learners may complete modules in any order and at their own pace. Each section includes links to additional information and resources.
Oregon clinic team members, administrators, quality improvement professionals and other primary care stakeholders may use these modules to:
- explore PCPCH recognition for the first time
- revisit the standards in advance of a verification site visit from the Oregon Health Authority; get a refresher on the model and standards
- or to orient new staff
