What is Do My PART?

Do My PART is focused on helping patients and their families and caregivers to take steps to improve the hospital discharge process and actions to improve the transition from hospital to home.

This resource center provides materials designed to help you communicate more effectively with your healthcare providers during the discharge process and fully understand your role in self-care and follow-up activities after leaving the hospital.

A variety of resources and materials are available to help patients and caregivers appropriately plan for a variety of scenarios regarding a transition of care:

  • A hospital visit and subsequent discharge
  • An effective transition of care to home or another care setting
  • At-home care following a hospital stay

The ultimate goal of this program is to avoid an unnecessary and costly readmission to the hospital.
