The Utah Million Hearts Coalition, in conjunction with the national Centers for Disease Control Million Hearts® initiative, aims to prevent heart attacks and strokes by improving blood pressure measurement and control. The coalition is working toward this goal by encouraging and recognizing excellence in hypertension management. The current focus is on improving the accuracy of blood pressure measurement in Utah.
High blood pressure is a leading cause of heart disease and stroke in the United States, and contributes to almost 350,000 deaths annually. Measuring blood pressure accurately is the first step in improving hypertension management.

The Utah Million Hearts Coalition Excellence in Blood Pressure Measurement and Control Award is the premier award in Utah recognizing the hard work, dedication, and outstanding achievements of clinics in Utah that focus on excellence in blood pressure measurement and hypertension control. This award acknowledges the effort and commitment clinics have made to meet the highest standards of clinical blood pressure measurement. The award is tiered from Bronze to Platinum with the opportunity to improve each year.
The award application is open from Feb. 1, 2018 to April 2, 2018. Winners will be announced and awards distributed by May 1, 2018.
Apply here and answer the one page application survey. The application process is designed as an education and resource tool to give providers the most current information that they may incorporate into their practice. The first 60 qualified clinics to apply will be eligible for a $500 stipend.
Award recipients receive: Certificate, recognition web badge and media package, and are eligible for a $500 stipend.
