An EHR can help you identify, diagnose, and follow up with a group - or population - of patients (hence the term population management). Looking at trends within your patient population can help your clinic determine common issues that may be impacting a large group of people, such as a high no-show rate or poor blood pressure control. It can be difficult to see these trends over time without an EHR. Many EHRs also have the ability to alert the provider to an elevated reading. These resources aim to help you use your EHR in your improvement efforts.
A study published in Annals of Family Medicine. The study purpose was to “develop a technology-based strategy to identify patients with undiagnosed hypertension in 23 primary care practices and integrate this innovation into a continuous quality improvement initiative in a large, integrated health system”.
