(HI) When patients are diagnosed with a chronic disease, health professionals often provide initial health education and have little capacity for further support. Ongoing support and reinforcement of self-care skills can be provided by community-based resources including evidence-based self-management workshops. This webinar will discuss the process of patient referral to self-management programs as an important element of care coordination, which can improve patient satisfaction, engagement and health outcomes, and will include ways to integrate referrals into clinic workflow.

(HI) This year's MDS 3.0 RAI User's Manual update was packed full of changes, from a new item set (the Medicare Part A PPS Discharge Assessment), to the new section GG on the MDS. These changes are big! This session will focus on detailing the rules according to final instructions from CMS. We'll discuss clarifications and changes from CMS on how to accurately capture items in section GG such as \"usual performance\" and \"helper,\" and when to complete the End of Medicare Part A Assessment. In addition to the big changes, the session will clarify numerous miscellaneous changes to chapters 2 and 3, including MDS sections C, J, M and others.

This event provides an overview of the report and explains how to interpret and use the information.

2015 Annual QRURs show how groups and solo practitioners performed in 2015 on the quality and cost measures used to calculate the 2017 Value-Based Payment Modifier (Value Modifier) and how the Value Modifier will be applied to physician payments. The federal Quality Measures continue to be the focal point of the facility report card on Nursing Home Compare and serve as a baseline for developing value-based payment systems for Medicare. With new QMs recently added and more on the way, having a clear understanding of how QMs are captures and how to build a strong QM program is more important than ever. (HI) Shared decision making (SDM) is a collaborative process that allows the patient and provider to make important health care decisions together. It takes into account the best clinical evidence available as well the patient's preferences for care. Patient decision support tools, also known as decision aids, are evidence-based tools used to help patients understand their condition and the preference-sensitive decision that needs to be made.

Implementing SDM into routine clinic processes can be challenging. During this presentation by Megan Bowen, compliance specialist and site visitor with the Oregon Health Authority's Patient Centered Primary Care Home Program, you'll learn about some valuable lessons from the field, how to choose the most appropriate SDM topics for your clinic, how to integrate patient decision support tools and the process of SDM into routine workflows, training resources for providers and staff, and where to get additional resources.

The RAI process harnesses resident voice through MDS Sections C, D, F and J, but is often underrated in its positive impact on resident outcomes. The data in these sections are impacted by social services, activities and nurses, but how often do all three disciplines collaborate in coding? The MDS Part 2 session will cover Section C – Accurately coding the gateway questions, determining cognitive status, managing care of residents with dementia; Section D – Capturing and managing mood indicators; Section F – Using resident preferences to create person-focused care plans; Section J – Identifying and managing pain.
