This National Diabetes Education Program webinar features three speakers who have delivered effective diabetes and prevention education to underserved populations. Each speaker comes from different settings: food banks, Area Agency on Aging/disability resource center and federally qualified health center/primary care.
Participants will hear how diabetes prevention and management is being addressed through community collaboration in rural, urban and high-risk/high-burden settings. Presenters will describe how they built and sustained diabetes management and prevention collaborations in their communities, including information on:
- Identifying the challenges and opportunities unique to the setting
- Identifying partners (organizations, government, faith, business)
- Planning collaborative work
- Evaluating community collaborations
