Webinar: Community Collaboration to Prevent and Manage Diabetes

This National Diabetes Education Program webinar features three speakers who have delivered effective diabetes and prevention education to underserved populations. Each speaker comes from different settings: food banks, Area Agency on Aging/disability resource center and federally qualified health center/primary care.

Participants will hear how diabetes prevention and management is being addressed through community collaboration in rural, urban and high-risk/high-burden settings. Presenters will describe how they built and sustained diabetes management and prevention collaborations in their communities, including information on:
  • Identifying the challenges and opportunities unique to the setting
  • Identifying partners (organizations, government, faith, business)
  • Planning collaborative work
  • Funding
  • Evaluating community collaborations
