Reminder of Hospital IQR Program Deadline

The purpose of this email is to remind hospitals participating in the Inpatient Quality Reporting (IQR) Program of the May 15, 2017 data submission deadline for the following:
·         Fourth quarter (4Q) 2016 Chart-Abstracted Clinical measures
·         4Q 2016 Healthcare-Associated Infection (HAI) measures
·         4Q 2016 – first quarter (Q1) 2017 Healthcare Personnel (HCP) Influenza Vaccination measure
·         4Q 2016 Perinatal Care (PC-01) measure
·         Calendar year (CY) 2016 Structural measures
·         Fiscal year (FY) 2018 Data Accuracy and Completeness Acknowledgement (DACA)

By May 15, 2017, at 11:59 p.m. PT, IQR-eligible hospitals are required to:
·         Upload clinical data through the QualityNet Secure Portal (https://cportal.qualitynet.org/QNet/pgm_select.jsp)

Note: Hospitals that do not have an Emergency Department (ED) may file an exception using the IPPS Measure Exception Form. Otherwise, hospitals that do not have an ED must submit ED files each discharge quarter.

·         Submit HAI and HCP Influenza Vaccination data through the National Health & Safety Network (NHSN) application (www.cdc.gov/nhsn/login.html)
Note: Qualifying hospitals may file an HAI measure exception using the IPPS Measure Exception Form.

·         Complete the PC-01 submission using the Inpatient Web-Based Measures data entry tool via the QualityNet Secure Portal (https://cportal.qualitynet.org/QNet/pgm_select.jsp)
Note: Hospitals that do not deliver babies may submit an IPPS Measure Exception Form. Otherwise, hospitals that do not deliver babies must enter a zero (0) for each of the data entry fields for each discharge quarter.

·         Complete the Structural measures and DACA using the Inpatient Structural Measures/DACA data entry tool through the QualityNet Secure Portal (https://cportal.qualitynet.org/QNet/pgm_select.jsp)

To verify the status of your clinical/HAI data submission, you may run your Provider Participation Report, Case Status Summary Report, and other applicable reports. To verify the status of your PC-01 submission, you may run your Provider Participation Report or check your Inpatient Web-Based Measures summary screen. To verify the status of your Structural measures and DACA, you may run your Provider Participation Report or check your Inpatient Structural Measures/DACA summary screen. Please see the 4Q 2016 Hospital Inpatient Quality Reporting (IQR) Checklist for further information.

Important: Hospital Contact Information
To ensure your hospital receives critical communications about meeting the requirements of the IQR Program (and other CMS quality reporting programs), including submission-deadline reminders and program updates, it is important that we have the complete contact information for the key roles at your hospital. Updates to your contact information can be submitted, if needed, using the Hospital Contact Change Form.
