Shared decision making (SDM) is a collaborative process that allows the patient and provider to make important health care decisions together. It takes into account the best clinical evidence available as well the patient's preferences for care. Patient decision support tools, also known as decision aids, are evidence-based tools used to help patients understand their condition and the preference-sensitive decision that needs to be made.

Implementing SDM into routine clinic processes can be challenging. During this presentation by Megan Bowen, compliance specialist and site visitor with the Oregon Health Authority’s Patient Centered Primary Care Home Program, you'll learn about some valuable lessons from the field, how to choose the most appropriate SDM topics for your clinic, how to integrate patient decision support tools and the process of SDM into routine workflows, training resources for providers and staff, and where to get additional resources.
