Utah Home Health Agencies Named Recipients of 2016 HealthInsight Quality Award

Program Encourages Excellence in Patient Care

Salt Lake City — Utah home health agencies are being recognized for their commitment to improving and promoting patient-centered care.

HealthInsight, a nonprofit, community-based health care collaborative working to improve health and health care for patients and providers, is presenting the 2016 HealthInsight Quality Award to home health agencies across the state today.

Home health agencies are honored if they complete a quality improvement project aimed at improving patient-centered care and for demonstrating high-quality health care and excellence in performance on publicly reported quality of care measures and other specified quality markers. These measures come from data that are voluntarily reported by home health agencies across the nation. The national home health rankings used to select recipients are available by visiting www.healthinsight.org.

"We are pleased to recognize these outstanding home health agencies that are continually working to improve health care quality and patient care for the residents of Utah," said Juliana Preston, executive director, HealthInsight Utah. "Their commitment to excellence in providing quality health care delivery is evidenced by their achievement of this award."

The award recipients will be recognized at the Utah Association for Home Care Holiday Award Luncheon at Le Jardin in Sandy, Utah.

The 2016 HealthInsight Quality Award recipients are:

  • Aspire Home Health — Taylorsville
  • Caregiver Support Network — Murray
  • First Choice Home Health and Hospice — Orem
  • Hearts for Hospice & Home Health — Ogden
  • Midas Creek Home Health & Hospice — South Jordan
  • Millcreek Home Health and Hospice — Salt Lake City
  • Pinnacle Home Health, Inc. — Holladay
  • Sunrise Home Health — Provo
  • Superior Home Care and Hospice — Murray
