Healthcare Providers

Do My PART strives to help healthcare providers in improving communication with patients during the discharge process by increasing patient engagement. We offer a number of tools that can assist in improve communication with patients and encourage patient-centered discharge planning. These tools can be printed, and in some cases, customized to your facility.

Patient Engagement

The Agency for Healthcare Research and Quality (AHRQ) defines patient engagement as a "set of behaviors by patients, family members and health professionals and a set of organizational policies and procedures that foster both the inclusion of patients and family as active members of the health care team and collaborative partnerships with providers and provider organizations." Greater engagement of patients, families and the public has the potential to transform every dimension of health care.

This online resource center is also a tool you can share with your patients to help them understand their role in staying healthy.
