Excellence in Blood Pressure Control Award

    Chances are you already provide excellent care for your patients with high blood pressure, and we know you want to continue to deliver the best up-to-date care available. To acknowledge all the hard work your team puts into high blood pressure care, and to share valuable updates on blood pressure care, we encourage you to apply for the Excellence in Blood Pressure Control Award.

    Applying for the award is easy, all you need is 20 minutes and two pieces of documentation (if available):

    • A report with your blood pressure control rate **please remove any patient identifiers**
    • A sample patient visit summary from your EHR with blood pressure care recommendations

    You must complete the entire form at one time, as your application cannot be saved. We recommend using Chrome or Firefox for this application. You may need to gather input from team members within your clinic, including an MA, Office Manager, and Physician.

    Award applications are due April 1, 2017. Award recipients will be announced by June 1, 2017

    Possible levels with points:
    Bronze: 26-29 points (65%-74%)
    Silver: 30-33 points (75%-84%)
    Gold: 34-37 points (85%-94%)
    Platinum: 38-40 points (95%-100%)

  4. 01. Enter your overall hypertension numerator and denominator in the provided boxes, preferably using NQF 0018 Controlling High Blood Pressure report numbers. The report for this measure should be 90 days or more starting after January 1, 2016. The denominator in your report must be a minimum of 100 patients and, as this is a clinic-wide award, the denominator should include patients from all providers.
